An individual with exceptional organizational and interpersonal skills to join our team as a Consumer Care Coordinator in our non-skilled home care agency.
Key Responsibilities Care Coordinator:
Consumer Assessment and Planning:
Conduct thorough assessments of consumers' needs, preferences, and goals.
Develop personalized care plans in collaboration with the consumer and their family.
Communication and Documentation:
Serve as the primary point of contact for consumers and their families.
Maintain clear and accurate records of consumer information, care plans, and any changes in status.
Resource Allocation:
Efficiently allocate available resources to meet the needs of consumers, ensuring cost-effectiveness while maintaining high-quality care.
Monitoring and Reporting:
Regularly monitor consumer progress and well-being.
Provide timely updates to the management team, addressing any concerns or changes in consumer status.
Advocacy and Support:
Act as an advocate for consumers, ensuring their preferences and rights are respected.
Provide emotional support and resources to both consumers and their families.
Consult with consumers and family members to discuss their home care needs.
Educate consumers about their condition and give them specific instructions.
Develop an individualized care plan to address their personal home care needs.
Consult and collaborate with other home care providers to set up consumer appointments and care plans.
Assist the care team with developing and assessing home care interventions.
Treat consumers with empathy and respect and conduct oneself in a professional manner.
Comply with agency guidelines and Delaware State home care laws and regulations.
Assist employees in understanding their duties, responsibilities, and the Agency's operating procedures and rules.
Perform 90-day follow-up visits and assessments of consumers and document their progress.
Conduct in-home assessments for new consumers as required.
Conduct oversight visits to supervise and evaluate direct care workers.
Audit Consumer charts on an ongoing basis Report missed visit to designated parties on a weekly basis Ability to travel within the three (3) counties for consumer assessments as needed.
Work under the direction of a supervisor and team lead Education/Training/
Experience:
Bachelor's degree in Health or a relevant field is a must; additional relevant certifications are a plus.
A minimum of one year of home care supervisory experience.
Proficiency in conflict resolution and the ability to find effective solutions.
Demonstrates a compassionate and kind demeanor.
Strong multitasking and task prioritization abilities.
Capable of making decisive decisions while maintaining discretion.
Possesses superior organizational and interpersonal skills.
